In consideration for releasing this information, I irrevocably and unconditionally release and forever discharge NPC, and each of its directors, officers, employees, and agents, including predecessors, successors, and assigns from any and all actions, causes of action, suits, debt, claims, complaints, liabilities, and demands of any nature, in law or equity, that I ever had, now have, or hereafter may have, by reason of any claims relating to the release of the information so designated in this document, including but not limited to any claims that I may have for defamation, slander, libel, and tortious interference with contract or business.

I have read all the terms of this Waiver and sign it voluntarily, and accept all obligations contained in this Waiver in exchange for the mutual promises contained in this Waiver and the consideration above.
